Terms & Participation Waiver
Participation Agreement
By booking a session with PBT Futbol, the parent or guardian acknowledges that soccer training involves physical activity and inherent risks of injury. Participation is voluntary and the parent/guardian accepts responsibility for the player's participation.
Injury Acknowledgment
The parent/guardian acknowledges that soccer training involves physical exertion and contact with balls, equipment, and other players in group settings. PBT Futbol takes reasonable precautions to minimize injury risk but cannot guarantee injury-free participation. In the event of injury, PBT Futbol will follow the emergency action plan and contact the emergency contact provided during intake.
Medical Information
The parent/guardian agrees to provide accurate and complete medical information during intake, including injuries, allergies, and medical conditions. Failure to disclose relevant medical information releases PBT Futbol from liability related to undisclosed conditions.
Cancellation Policy
Sessions cancelled 24 hours or more in advance receive a full credit toward a future session. Sessions cancelled with less than 24 hours notice may forfeit the session deposit. Weather cancellations are always rescheduled at no cost. PBT Futbol reserves the right to cancel sessions due to unsafe weather conditions, field unavailability, or coach illness, with full rescheduling at no cost.
Payment Terms
A deposit is required to confirm each session booking. Full session payment is due before or at the start of the session unless a package arrangement has been made. PBT Futbol reserves the right to cancel unconfirmed bookings.
Media Consent (Optional)
Media consent for photos and videos is separate from session participation and is entirely optional. If consent is not provided, no photos or videos of the player will be taken or used. Consent may be withdrawn at any time.
Code of Conduct
Players and parents are expected to maintain respectful conduct during sessions. PBT Futbol reserves the right to end a session and decline future bookings in the event of disrespectful, unsafe, or disruptive behavior.
Governing Law
These terms are governed by the laws of the State of Indiana. Any disputes shall be resolved through good-faith communication before any formal action.
